ETIAS application form: expected questions and safe preparation
There is no live official ETIAS application form yet. The official preparation page shows the categories the future form is expected to request: identity and contact details, passport information, occupation and education, intended first stay, specified declarations, and representative details where another person applies. Use this page as a field guide only; do not enter or upload personal data here.

Expected ETIAS form field map
The live wording may change. These categories come from the official preparation material and should be rechecked at launch.
- Identity
- Names, date and place of birth, sex, nationalities, home address, email, phone, and parents' first names.
- Travel document
- Document type and number, issue and expiry dates, and issuing country or authority.
- Work and education
- Current occupation and level of education.
- Trip
- First intended country of stay and destination address or related itinerary information.
- Declarations
- Specified convictions, conflict-zone travel, and return decisions within the published scope.
- Representative
- Identity, relationship, organization details where relevant, and confirmation of a signed representation declaration.
The accuracy pass before future submission
Most preventable form problems come from mismatches between the passport, itinerary, and entered data.
- Use the same passport that will be presented for travel.
- Copy names, document number, dates, and issuing details exactly from the document.
- Use an email address the traveller controls and will monitor.
- Check the first-stay country against the actual itinerary rather than the flight connection alone.
- Answer declarations from the official wording; do not rely on a third-party paraphrase for an exceptional case.
- Save the reference and confirmation only after the official system is verified as open.
Families, minors, and representatives
A family trip does not create one group ETIAS. The future form is expected to remain individual.
- Children
- Each child needs an individual application linked to that child's travel document; a fee exemption may apply.
- Another person applies
- The form is expected to collect representative and relationship information plus a signed declaration.
- Commercial intermediary
- It must disclose identity, optional status, fees, privacy terms, and scope separately from the official fee.
How to avoid a fake or premature form
A page can describe ETIAS accurately and still have no authority to collect an official application.
- No site can submit a live official ETIAS while the EU says applications are not being collected.
- Start from travel-europe.europa.eu rather than an advertisement or lookalike domain.
- Do not upload a passport or pay an official application fee before the official channel opens.
- Return to the main application guide for the operating-state and launch workflow.
